Care Partnership Office

The Care Partnership Office was created in 2020 to support and sustain efforts to actively partner with patients, residents and family caregivers through embedded practices across the organization. The office’s mandate includes a strategic direction to ensure ongoing development, research and innovation in the area of care partnership work at St. Joseph’s. From an operational standpoint, the office focuses on:

• Capacity building within the organization:
  • Championing high quality engagement and co-design practices
  • Providing tools, resources, education, consulting and mentoring to assist leaders, staff and physicians in the practice of patient, resident and family caregiver engagement
• Management of Patient and Family Partners
  • Patient and Family Partner recruitment, onboarding, matching and stewardship
  • Support of patient and family councils
• Measurement and Evaluation
